We believe that effective collaboration and communication between separated parents are essential for your child's welfare, including their health, education, social life, and a mutually agreed-upon parenting framework.
We emphasize working closely with both parents to ensure the well-being of the child and minimise conflict.
Responsibilities and Commitments
This page outlines the responsibilities and commitments we believe are crucial to manage your child's care and respect the rights of both parents. For us to provide the best care for your child, we require both parents to agree to the following terms, unless one parent has sole custody for decision making:
- Consent to assessments and any agreed-upon therapy being conducted at Ascot Vale Paediatrics for the care of my child.
- Commitment to being courteous and civil during all consultations.
- Agree that all bills are to be paid at the time of the appointment, and any financial arrangements or Medicare rebates will be subsequently negotiated between the parents. Reception staff will not be involved with any arrangements. The parent arranging the appointment is responsible for payment, unless court orders advise otherwise.
- Understand that if there is a risk of harm to the child or a legal obligation to report a disclosure to the Department of Family and Community Services, it will be done.
- Acknowledge that in cases of joint parenting, it is the parent who arranges the appointment's responsibility to inform the other parent of the appointment time and seek their availability before scheduling the appointment.
- Understand that copies of correspondence will be sent to both parents if both parents are present for the appointment. If only one parent attends, it is the attending parent's responsibility to share correspondence with the other parent.
- Understand that Ascot Vale Paediatrics may disclose relevant information provided by either parent in our reports.
- Questionnaires and forms must be completed by each parent (unless one parent has sole custody for decision making). This is to ensure we have a full understanding of your child's behaviour and abilities in each of their homes.

• Our separated parents consent form (as per above) signed by both parents, unless court orders advise otherwise
• Copies of any court or parenting orders regarding medical decision making for the child.

Ascot Vale Paediatrics reserves the right to refuse care, assessments, or any service if the above terms are not honoured or if either parent's behaviour is detrimental to the child's welfare.
